Greetings Vikings,

Starting next week, we will begin distributing materials and chromebooks to Smith students who need one and did not receive one in March.

In order to have a chromebook ready for your child, please help us out, by completing the short survey below. Students who received a chromebook in March, may keep it for the start of the school year and will NOT be issued a new chromebook. If you have more than one child in need of a chromebook, please complete this form for each child by Friday, August 21.

IF YOUR CHILD DOES NOT NEED A CHROMEBOOK, YOU DO NOT NEED TO COMPLETE THIS FORM.
