A message from, Ms. Sisty, the Athletic Director at Smith Middle School:

As we have all learned through this strange time, things change quickly and revisions are made, sometimes often, based on feedback. We listened to all of the parent/guardian suggestions and concerns, and have revised the PE waiver. Please make sure to read through the attached waiver completely. This waiver will need to be signed by all parents/guardians of students who will be attending PE classes. Please use one of the multiple ways below to return the PE waiver to Smith Middle School:

• Print, sign, and fax to 313-827-2815
• Print, sign, and scan to sistye@dearbornschools.org
• Print, sign and bring to the main office between 8am-3pm (For your convenience, the PE waiver is also available in the office.)

The new PE waiver will need to be returned as soon as possible! To help ensure that the PE waiver gets to the correct PE/Health teacher, please write the teacher’s name on the top of the waiver.

**If you have already turned in the previous waiver you are all set, and will not need to complete the new revised waiver unless you would like to do so.**
