Dentists-R-Us will be at Smith Middle School on Thursday, June 2nd. The Licensed Dentists will be available to perform:

  1. Comprehensive Dental Exams
  2. Prophylaxis (Dental Cleaning)
  3. Fluoride Treatments
  4. Sealants (When necessary)

Each participant will receive a free toothbrush and an oral health report will be sent home to the parent. If you would like your child to be seen, please fill out the permission slip and return it to the office by Friday, April 29th. Permission Slips were given to Smith students during their Advisory class also, however you may print the attached document as well.

Please note that students who had a cleaning within the last 6 months would NOT be eligible.
